ブータン滞在終えて

秋にブータンへ行ってきた。初期研修の時に、ブータンに興味を持ち、フィールド医学を志してから6年越しの実現だった。

滞在したのは、首都から東に車で2,3日かかる、タシガン、ツァンポという村。太陽の降り注ぐ、自然豊かな、鍵のいらない、誰がどこに住んでいるかわかる村。レストランがない分、お宅へおじゃまする機会は多かった。

食事は、主食はお米、それに唐辛子がスパイスではなく、野菜として毎食用いられている辛いカリーがつく。お酒はビールに、農家がトウモロコシから作った焼酎。その焼酎を、油で炒めた卵に注いで飲む。体がすぐに熱くなるが、ビールより好きだった。オーガニックだと言うが、体にいいのか悪いのかは不明。

滞在の目的としては、診療所で診療しながら、ブータンの方々の心の持ちよう、文化を知ることであったため、たくさんのことにトライした。食べ物、お酒、フットボール、祭り、踊り。そして、診療所にくる口腔内や皮膚異常の子たちから、食事によるビタミンやミネラル不足が関連しているのではないかと考え、歯科医と協力して学校健診に参加し、食生活と症状の関係を探ってみた。

2か月間、村につかり、歩いていても、皆見覚えがあり、村を心配するという感覚に触れられた。以前、地域医療はその場所に住むことだと教えていただいたのが頭浮かんだ。村全体が大きな家族であり、感情を共有し、悲しみは半分に、楽しみは2倍になっていた。

診療所の仲間はどんなキャラクターかつかむことができ、おそらく、自分がどんな人間かもわかってもらえたかもしれない。そのような関係性ができたことで、文化関係なく、人としての基本を再認識した。

人は人でもちこたえている。一緒に食べて、飲んで笑う、約束を守る、手間を評価する、感謝する。壁を作らずに、聴く器量を持ち、共感を生むこと。

また、大きな自然を前に、自然の一部であることを実感させられる。私たちは、自然を利用させてもらっており、太陽の暖かさはとてもありがたいものだと感じられた。

ブータンに行かなくても感じられることかもしれないが、今ブータン、ツァンポにいたからこそ、そこにいる人、自然から教えてもらえた。偶然であり必然なのだろう。これからも愛している。

今後ともブータンにかかわる機会をいただけそうなので、カルテ管理、そして文化に沿いつつ、個別の生活習慣から飲酒、食生活、痛み、孤立予防、うつにかかわり、全体に波及できればと考えている。

片頭痛や胃炎が多そうだが、そこも頭に入れながら。本当にありがとう。

I went to Bhutan. At the time of the initial training, I became interested in Bhutan and realized it over six years since I started pursuing field medicine. I stayed in a village called Tashigan, Tsangpo, which takes a few days to drive east from the capital. A sunny village, rich in nature, no key, a village where you know who lives. Since there was no restaurant, there were many opportunities to visit their house. Meals are made with rice and spicy curry, chili is not used as a spice, but as a vegetable. Liquor is beer and local made from corn by farmers. Pour the local into a fried egg and drink it. I got hot soon, but I liked it better than beer. Says it is organic, but it’s unclear whether it’s good or bad. The purpose of my stay was to know the culture of the Bhutanese people while having medical treatment at the clinic, so I tried many things. Food, alcohol, football, festivals and dances. Then, I thought that vitamins and mineral deficiencies due to meals might be related to children with abnormal oral and skin coming to the clinic, and participated in school health checkups in cooperation with dentists. I tried to explore the relationship. Even if I walked into the village for two months, I felt that everyone was familiar and worried about the village. I was reminded of the fact that community medicine is about living in the place. The whole village was a big family, sharing emotions, grief halving and fun doubling. I can bite their character and perhaps they know what kind of person I am. By having such a relationship, I reaffirmed my basics as a person without cultural relations. People hold up with people. Eat together, drink and laugh, keep promises, appreciate effort, be grateful. Have the ability to listen and create empathy without making walls. Also, in front of the great nature, I can feel that people are part of nature. We used nature and felt that the warmth of the sun was very appreciated. It may be felt without going to Bhutan, but because I was in Bhutan and Tsangpo at that time, I was taught by the people there and nature.It will be accidental and inevitable. I love you in the future. I am sure that I will have opportunities to get involved in Bhutan in the future, so I hope to be able to spread from the individual lifestyles to drinking, and eating habits, pain, isolation prevention, and depression in line with culture. I keep it in my head, the medical chart using the cloud, thanks a lot.
